All athlete medical forms must be completed and reviewed by the registrar before the athlete may participate in any Row New Jersey competitive racing program. Learn-to-Row participants are not required to submit a physical.
***Required Health/Medical Documentation for Participation***
All New & Returning Athletes are required to have a current physical on file. Please have your physician complete the NJ Preparticipation Athletic Physical Form which can be found on our website under Rowing 101, forms & files. CVS/Quick med type physical forms are not accepted.
Additionally - If the athlete is returning but has HAD ANY MATERIAL CHANGES IN THEIR HEALTH OR HEALTH CARE, a ROW NJ Health Update Form must be signed by the athlete's parent and physician. It too can be found on our website under Rowing 101, forms & files.
All athlete medical forms and waivers must be completed and reviewed by the registrar before the athlete may participate in any Row New Jersey programs. Each season athletes must upload current documentation to the applicable season Medical Form Portal (even if you uploaded for past seasons). For security purposes the system deletes the documentation after the season end date, therefore you must upload for each season. If your physical is not uploaded prior to the start date, you may not be able to participate in training.
We do understand that you may need to provide an updated physical mid-season. Prior to 3/6/23(season start date), please upload your current physical completed on the required form ( NJ Preparticipation Athletic Physical Form). Once your athlete has a new physical examination and you receive your updated form, you may log back in to the portal and upload the latest version of your physical. (example: Simba Smith’s last physical was on 3/17/22, therefore is current/valid until 3/17/23. Their next physical is scheduled for 3/18/23 and they receive an emailed copy of the completed NJ Preparticipation Athletic Physical form back from the physician’s office on 3/21/23. Simba’s parent/guardian shall log back into this season’s upload portal and provide the latest forms. It is suggested that you create a folder on your personal device for ease each season. We appreciate your cooperation.
